A Case of
Chronic Localized Tetanus in an Immunized Patient

HPI / Case Details: Patient is a 57 year old female physician with past medical history of high cholesterol, celiac disease and multiple food allergies who sustained an open crush fracture of the left big toe in June 2022 after a heavy double glass and metal shower door fell on her toe. She was seen in the ER and received wound care with tetanus booster and trephination of toenail.  2-3 days after injury intermittent twitching was noted in the small muscles surrounding the injured toe. One week after injury, twitching worsened around the medial left foot, progressed to the lateral toes of the left foot, then gradually progressed further up the left leg. Once twitching reached the gluteal muscles (ascending spread), it spread to the bilateral extremities and back. There were several strong tonic contractions of gluteal muscles which lifted the patient off of the couch involuntarily. At this time patient had concern for tetanus and sought initial ID consult. Labs done included Creatinine Kinase which was 602 with normal CBC, CMP and negative blood cultures.

  

Initial ID consult diagnosed local tetanus based on clinical symptoms and recommended PO Metronidazole 500mg every 6 hours as an outpatient. Autonomic involvement developed with tachycardia and cardiac arrhythmia, along with episodes of sudden sweating. Intermittent twitching continued to be present bilaterally in the body, face, jaw &  tongue. One episode of diaphragmatic spasm was noted. Loud noises exacerbated twitching / spasms. Symptoms were suppressible with benzodiazepine. Tetanus IM IG 500 IU was recommended at this point and obtained. 

  

Patient was hospitalized in August 2022 for IV metronidazole which helped reduce symptoms for about one month. MRI of the left toe showed still healing fracture and ortho consult did not recommend surgical intervention at that time. Multiple neurology consults were obtained to rule out neurological cause of symptoms given the unusual presentation and all neurology consults felt this was not primarily a neurological issue and she was referred back to Infectious Disease for management.  EMG & MRI of the brain were done as part of the neuro workup and were both normal. (Patient was advised to continue benzodiazepine during initial EMG so any possible exaggerated F-response and spasms would have been suppressed by benzodiazepines during EMG if present). During all physical exams involuntary muscle spasms/ twitching of the left foot/toes were noted involving both agonist and antagonist muscles.

  

A second Tetanus Vaccine (Tdap) was recommended 4 weeks after the initial tetanus booster. Three days after the second tetanus vaccine, patient developed new onset full body pins & needles and later developed into sensory peripheral neuropathy in bilateral feet & left hand which neurology felt was likely due to excess tetanus vaccine related side effect since there was partial response of new onset sensory neuropathy to monthly IVIG treatments which were started to treat the post vaccine new onset sensory neuropathy.

Tetanus antibody levels consistently show patient is fully hyperimmunized (>7 high end of assay) Tetanus antibody levels have not decreased with time over 26 months, even though diphtheria antibody levels (given in the same Tdap vaccine) have steadily declined with time as expected. (Suggesting continued immune exposure to tetanus toxin)

  

A trial of Hyperbaric oxygen was attempted to aid in fracture healing (with the thought that clostridium tetani would not remain active once fracture was fully healed) Hyperbaric oxygen helped reduce local twitching in the left foot and reduced generalizations and autonomic involvement which would occur with walking or increased pressure on the left great toe. (Throughout this time patient could not walk more than small amounts without spasms/ twitching generalizing up the left leg from the left foot) (In the literature Tetanus toxins (tetanospasmin and tetanolysin) have been noted somewhat oxygen labile.) Hyperbaric oxygen was discontinued after patient developed cataracts as a side effect of 6 months of hyperbaric oxygen treatments.

  

Local muscle twitching around the left toe/foot worsened off hyperbaric oxygen when patient walked more than very small amounts. Symptoms always began with hyperesthesia in the left big toe, muscle twitching progressing up the left leg incrementally, then once twitching had reached the spinal level, muscle twitching spread to the bilateral muscles of the body with cardiac arrhythmia & autonomic involvement/ sweating. A trial of antibiotic Keflex was suggested, which at high dose (1000mg PO every 6 hours), helped keep spasms localized to the left foot mostly but symptoms would worsen and generalize if antibiotic Keflex was stopped for more than 2 days. Benzodiazepine was increased to every q4-6 hours to reduce muscle twitching that had become constant in the left foot/ toes. Symptoms improve when patient does not walk or avoids any pressure on left big toe and symptoms worsen when patient walks or stops antibiotic and has been unable to stop antibiotic for the past year and a half without worsening of symptoms. Bilateral jaws begin to snap shut intermittently involuntarily when antibiotic is stopped for 2 days and cardiac arrhythmia/ heart block reoccurs.  Symptoms worsen further 4 days off antibiotic. 

  

Repeat MRI one year after injury showed some abnormal signal. Clostridium tetani infection generally has no clear radiological findings or lab confirmatory tests and is generally a clinical diagnosis. Patient sought consults with multiple infectious disease doctors in New York after initial diagnosis but despite repeated worsening off antibiotics, clear clinical signs of local tetanus in the left foot, progression of spasms / twitching up the left leg with generalization and autonomic symptoms including heart block and involuntary jaw closure within days of increased walking or when antibiotics are stopped, infectious disease physicians in New York (who have not had experience with tetanus) have been conflicted on the diagnosis and of little help. (Of note the medical literature reports that in an immunized patient the presentation would typically be different with an ascending pattern of spasms / twitching up the affected limb from the localized point of infection with progression to generalization.)

Patient sought surgical consult with a bone specialist surgeon who diagnosed patient clinically with tetanus osteomyelitis & surgical debridement of the left big toe (distal bone) was attempted 2 times. First surgery involved bone debridement of the medial side of the left big toe with placement of cefazolin antibiotic laden calcium stimulan beads. The first surgery did not resolve the symptoms. One episode of brief full left leg tetany occurred in the first month post surgery. After the post operative period spasms continued and would again worsen and generalize with cardiac involvement/ heart block and involuntary jaw closure if antibiotic was stopped or if patient walked more or put pressure on the left big toe.  During the surgery, the surgeon visually graded the bone as "infected" with an infection level of 4 out of 4, stating the bone appeared much softer than it should be. (Of note, the surgeon specializes in bone surgery and stated he had never seen bone that looked like this) Cultures (anaerobic, aerobic and fungal) were negative but the surgeon and literature advises that anaerobic cultures rarely come back positive (accuracy is only about 30% in positive cases).

  

A second bone debridement surgery was attempted on the lateral side of the left big toe. Metronidazole was discussed as being a  possible ideal antibiotic for the antibiotic laden calcium stimulan beads but surgeon advised pharmacy was unable to obtain the appropriate dilution of metronidazole for the beads  to be effective so vancomycin was used in the antibiotic laden calcium beads instead. Of note each surgery temporarily worsened twitching/spasms for one month post surgery requiring increased doses antibiotic and spasm suppressing benzodiazepine to control symptoms. (Speculation is that the clostridium tetani bacteria proliferates in cut (deoxygenated) tissue.) 

  

Plan during the first surgery was to submit lab samples for 21s rRNA PCR testing for clostridium tetani however the bone sample from first surgery was placed in the incorrect media so PCR testing of the clearly infected sample could not be performed at all.  Surgical sample from the second debridement surgery (taken from a different site on the toe) was noted to be visually "clean" during the surgery (without signs of infection). It was sent to Mayo Clinic for PCR testing and results were negative however infectious disease consult at Columbia Hospital states that Mayo clinic PCR results are often inaccurate in their opinion and they recommend if more samples are to be sent they should be sent to the University of Washington as they find their testing to be more accurate. Local Tetanus Symptoms and generalization when antibiotics are stopped continue to occur after the second surgery.

  

Next high dose Bicillin-LA Intramuscular injections (Benzathine Penicillin G) were suggested by an infectious disease physician. 2.4 Million units twice a week did not improve symptoms so it was recommended dose be increased to 2.4 million units three times a week for 2 weeks which did improve symptoms significantly. Oral Keflex was able to be temporarily discontinued (benzodiazepine was continued) and patient was able to walk more without exacerbation or generalization however approximately one week after 2 week course of Bicillin-LA was discontinued, spasms began to worsen again around the left toes and foot and progress up the left leg with generalization again.

Oral Keflex 1000mg 4 times a day & IM Bicillin LA 2.4 million units 3 times a week were restarted with improvement in bilateral body muscle twitching but twitching / spasms in locally in the left toes and foot continued. This regimen was continued for 7 weeks. One day after discontinuing oral Keflex (and 3 days after last bicillin IM shot) spasms again began to progress up the left leg to the left gluteal muscles and again once the spinal level was reached spasms spread to the right side of the body and then upper body and by 1 and 1/2 days after antibiotic was discontinued, arrhythmia/heart block began again. Oral Keflex 1000mg 4 times a day was restarted. Benzodiazepines had been continued throughout. 

Multiple attempts (>10) have been made to stop antibiotics over the course of treatment, consistently shortly after each time antibiotics are stopped the local spasms/twitching in the left toes / foot begin to generalize by progressing up the left leg, then spreading to the rest of the body once the spinal level is reached with subsequent jaw and cardiac involvement (heart block). No cardiac abnormalities or symptoms of generalization are seen while patient remains on antibiotic. This presentation appears consistent with multiple case reports & data below which has demonstrated that in an immunized patient, tetanus toxin takes a retrograde neuronal route before reaching the CNS and generalizing since the blood bourne route is blocked by antibodies if immunized. 

Surgeon has recommended allowing more time for bone healing while remaining on antibiotics and twitch suppressant medication  (it is now 2 and 1/2 years post initial injury/infection and patient still is not able to stop antibiotics without generalizing tetany) as at this point amputation may be the only option and it is unclear if the bacteria could spread further in the foot during the surgery.
